"FREE Dental Care For Life" Sweepstakes Official Rules

 

NO PURCHASE NECESSARY. Open to legal residents of the 50 U.S. States & DC, 18 years or older. Void where prohibited. Promotion starts at 12:00 a.m. ET on 11/15/08 and ends at 11:59 p.m. ET on 12/31/08. The following is intended for participation and viewing in the U.S. only and shall only be construed and evaluated according to United States law. Do not proceed if you are not a resident of the United States.

 

TO ENTER: Visit www.stopbrushing.com and follow the prompts to enter. Limit one entry per household, per day. Entries will be accepted from 12:00 a.m. ET on 11/15/08 to 11:59 p.m. ET on 12/31/08 (the "Promotion Period"). You can return to the website once each day during the Promotion Period to submit an additional entry into the sweepstakes drawing. When you enter for a chance to win, you will be eligible to win the grand prize which will be randomly awarded at the end of the Promotion Period. Grand prize sweepstakes drawing will be conducted on or about 1/15/09. Entrants must provide a valid e-mail address upon entry.

 

PRIZES: GRAND PRIZE: One (1) grand prize will be awarded and will include free dental care for life from Philips Sonicare (up to $25,000) in the form of a $25,000 check. Odds of winning depend on the number of eligible entries received. ALL EXPENSES, TAXES AND COSTS ASSOCIATED WITH RECEIPT OR USE OF THE PRIZE NOT SET FORTH HEREIN ARE SOLELY THE RESPONSIBILITY OF THE WINNER. No prize substitutions permitted by winner unless otherwise indicated. In compliance with IRS regulation, winners of any prize valued at over $600 will be required to provide his or her social security number and home mailing address for tax purposes.

 

GENERAL CONDITIONS: Employees of Philips Electronics North America Corporation, subsidiaries, advertising and promotion agencies ("Affiliate Entities") and their immediate families are not eligible. Grand prize winner must sign and return an affidavit of eligibility & liability/publicity release within 14 days of first attempted notification. Tennessee residents need not sign publicity release. Failure to provide signed documents will result in forfeiture of prize. The actual value of the grand prize is $25,000, awarded in the form of a check to be applied to prize winner's dental care at their own discretion. Sponsor, Administrator, prize provider, the Affiliate Entities and their employee agents are not responsible for any coordination or communication with dental professionals, nor for any diagnosis, complications, issues, negligence, etc. that may result from any dental visit and/or procedure. Prize winner must sign eligibility and liability documents prior to receiving the check.

 

ADDITIONAL LIMITATIONS: By entering, participants (a) agree to be bound by the official rules and the decisions of the Sponsor which are final and binding in all respects; (b) agree to release Sponsor, Administrator, prize provider, the Affiliate Entities and their employees and agents from any and all liability, loss, damage or injury resulting from participation in this promotion or awarding, receipt, possession use and/or misuse of any prize awarded herein and acknowledge that Sponsor, and its employees and agents have neither made, nor are in any manner, responsible or liable for any warranty, representation, or guarantee, expressed or implied, in fact or in law, relative to any prize including, but not limited to, its quality, mechanical condition or fitness for a particular purpose; and (c) consent to use of his/her name, photograph and/or likeness for advertising and promotional purposes in connection with this promotion without additional compensation, unless prohibited by law.

 

INTERNET: Sponsor reserves the right to terminate or modify this promotion if any causes beyond Sponsor's control impairs its integrity or fairness, in which case any un-awarded prizes will be awarded in a random drawing from all entries received as of the date of termination. Sponsor is not responsible for any problems or technical malfunctions of any telephone, network or lines, computer online systems, servers or providers, computer problems or traffic congestion on the Internet, or at any website, or any combination thereof including any injury or damage to entrant's or any other person's computer related to or resulting from entering the sweepstakes. Sponsor is not responsible for phone line failure and/or malfunction. CAUTION: ANY ATTEMPT BY A CONTESTANT TO DELIBERATELY DAMAGE ANY WEBSITE OR UNDERMINE THE LEGITIMATE OPERATION OF THE SWEEPSTAKES IS A VIOLATION OF CRIMINAL AND CIVIL LAWS AND SHOULD SUCH AN ATTEMPT BE MADE, SPONSOR RESERVES THE RIGHT TO SEEK DAMAGES FROM ANY SUCH OFFENDER TO THE FULLEST EXTENT PERMITTED BY LAW. In the event of a dispute as to the identity of a potential winner based on an e-mail address, the winner will be the authorized account holder of the email address submitted at time of entry. "Authorized account holder" is defined as the natural person who is assigned to an email address by an Internet access provider, online service provider or other organization (e.g., business, educational institution, etc.) that is responsible for assigning email addresses for the domain associated with the submitted e-mail address or phone number. Proof of being the authorized account holder may be required.

 

MARKETING PREFERENCES: By submitting a complete entry, you will have made marketing preferences noted to Sponsor, and may be contacted by Sponsor in accordance with these preferences. Upon receipt of any Sponsor contact, you may opt out of future contacts, and you will remain eligible to win the prizes.
